Terms of Service

Effective September 8, 2026.

FormFlorida.com is owned and operated by Form Florida LLC. References to “Form Florida,” “we,” “us,” or “our” in these Terms refer to Form Florida LLC.

These Terms govern purchases from Form Florida, a independent business filing and document-preparation service. Form Florida is not affiliated with the State of Florida, the Florida Department of State, Division of Corporations, Sunbiz, the IRS, or any other government agency. Form Florida is not a law firm and does not provide legal, tax, accounting, investment, or financial advice.

Services

Depending on the package purchased, Form Florida may prepare and submit Florida business formation filings using information supplied by the customer, provide administrative EIN application assistance, deliver templates, and order selected state-issued documents. The exact service purchased is the package description shown at checkout.

Customer responsibility and authorization

You are responsible for providing complete, current, truthful, and accurate information and for having authority to create or act for the entity identified in your order. You authorize Form Florida to use the information you provide to prepare and submit the administrative filing you request.

Government charges and processing

Government charges are disclosed separately from Form Florida service charges. Government agencies control approval, processing time, name acceptance, document issuance, and government refunds. Form Florida cannot guarantee approval or a specific completion date.

Legal and eligibility restrictions

You may not use Form Florida for fraudulent, deceptive, sanctioned, prohibited, unauthorized, or unlawful activity. Form Florida may refuse or cancel an order if it reasonably believes the requested filing is unauthorized, fraudulent, violates law, or cannot be supported. International access remains subject to applicable U.S. laws, sanctions, identity requirements, and government filing rules.

Public records

Information submitted to Florida government agencies may become public record. Do not provide unnecessary sensitive personal information for a public filing.

EIN assistance

The IRS does not charge a government fee to apply directly for an EIN. When EIN assistance is included, the customer is paying Form Florida for administrative assistance, not for an IRS filing fee. EIN procedures and eligibility can vary by applicant.

Digital delivery

Form Florida provides a digital service. No physical goods are shipped unless a future product page expressly states otherwise. See the Service Delivery Policy for expected workflow.

Payments and statement descriptor

Payments are processed by Stripe. Charges may appear on the customer’s card statement with a descriptor identifying Form Florida. Customers should contact support before opening a dispute so we can attempt to resolve any issue promptly; this does not limit cardholder rights.

Annual Compliance Plan subscriptions

The Annual Compliance Plan is a recurring annual subscription currently priced at $237.75 per year, consisting of a $99 Form Florida service fee plus the current $138.75 Florida LLC Annual Report state fee. The first annual charge is scheduled for January 1, and it renews each January 1 until canceled. If the applicable government fee changes, Form Florida will provide notice before changing the recurring subscription price. Late fees, reinstatement fees, amendments, and other government charges are not included unless expressly stated.

Refunds and cancellations

Refund and cancellation eligibility is governed by the Refund, Cancellation & Dispute Policy, which is incorporated into these Terms.

No guarantee of business outcome

Formation of an entity does not guarantee financing, banking approval, licenses, permits, tax treatment, immigration status, credit, profitability, or business success.
